'Like' and Win with Facebook!
So you've already 'Liked' our Facebook page at http://www.facebook.com/MyQ95FM, and now you're wondering... why should I have to fill out this form to be eligible to win movie tickets? Well, there are a couple reasons.

For one, many of our fans are from far corners of the state, from other states, and even other countries. Plus, even some people in the area just aren't interested. In the interest of opening the drawings only to those in the area who can claim their prize and make use of free movie tickets to theaters in Paintsville or Inez, we cut out a lot of headache this way.

Just as importantly, properly identifying our winners is crucial, and as a Page, Q95 FM has only access to the personal information that users choose to share publicly. That means in many cases, we can't gather phone numbers or even cities where the winner lives, and if two people have similar names, it makes things needlessly complicated. We'll never share any information you fill out in this form, and you'll be listed as a winner only by name and city, but it helps us verify our winners when they come to claim a prize and enables us to contact you if we need to.
