Jan Hackworth, age 74 of Swamp Branch, Kentucky, passed away Monday, July 12, 2021 at the Pikeville Medical Center.

Born April 27, 1947 in Magoffin County, she was the daughter of the late Clyde Lemaster and Allie Gamble.

Funeral services will be held 1:00pm Friday, July 16, 2021 in the chapel of the Phelps & Son Funeral Home with burial to follow in the Highland Memorial Park, Staffordsville. Friends may visit the Phelps & Son Funeral Home from 5:00pm to 9:00pm Thursday and anytime prior to the service on Friday. Robbie Lemaster will officiate. Arrangements are under the direction of the Phelps & Son Funeral Home.
