Floyd David Cook, age 70, of Louisa, KY passed away October 19, 2021 at Three Rivers Medical Center in Louisa, KY. He was born June 1, 1951 in Louisa, KY to the late Charlie and Lola Porter Cook.

Funeral services will be held on Saturday, October 23, 2021 at 1:00 PM in the chapel of Crum Funeral Home with John Goble officiating. Burial will follow in the Moore Family Cemetery on Debord. Friends may visit at the funeral home on Thursday and Friday from 5 PM to 9 PM. Arrangements are under the direction of Crum Funeral Home of Inez, KY.
