John Phillip Collinsworth age 82 of Salyersville, Kentucky passed away Sunday, June 19, 2022 at the Highlands ARH Regional Hospital in Prestonsburg, Kentucky.  He was born August 21, 1939 in Salyersville, Kentucky to the late Lawrence and Mallie Gilliam Collinsworth.

Funeral services will be conducted at 1:00 P.M. Friday, June 24, 2022 from the Holiness House of Prayer with Robert May officiating. Burial will follow in the Whitaker Cemetery at Guncreek, Kentucky. Friends may visit the church Wednesday, June 22, 2022 after 6:00 P.M., all day Thursday, and any time prior to funeral services.
