Annette Renee Blair, age 45 of Flat Gap, Kentucky passed away Friday, October 14, 2022 at Three Rivers Medical Center in Louisa, Kentucky. Born June 5, 1977 in Johnson County, Kentucky to Annette Denise Rowland Hayes of Flat Gap, Kentucky and the late Larry Hayes.

Funeral services will be held 1:00pm Tuesday, October 18, 2022 in the chapel of the Phelps & Son Funeral Home with burial to follow in the Highland Memorial Park at Staffordsville, Kentucky. Chris Rowland and Tim Salyer will officiate. Friends may visit at the Phelps & Son Funeral Home on Monday from 12:00-8:00pm and on Tuesday after 9:00am until time of services at 1:00pm. In lieu of flowers, the family request that contributions be made to the Phelps & Son Funeral Home to assist with funeral expenses. Funeral arrangements are under the direction of the Phelps & Son Funeral Home.
