Jay Cook, 23, of Belfry, KY, passed away Monday, October 25, 2021, at the Tug Valley Regional Medical Center, South Williamson, KY.  Jay was born April 19, 1998, at South Williamson, KY, the son of the late James Russell Cook and Staci Nicole Beckett.

Funeral services will be held Friday, October 29, 2021, beginning at 11 AM in the R. E. Rogers Funeral Home Chapel with Larry Blackburn officiating.  Burial will follow in the Curry-Bolin Cemetery, Turkey Creek, KY, with his family and friends serving as pallbearers.  Visitation will be held Thursday, October 28, 2021, from 6 – 9 PM in the funeral home chapel.  R.
